Background Multidisciplinary management in a combined clinic improves patient experience and outcomes for pregnant women with inflammatory rheumatic diseases (IRD). Achieving better IRD control pre, during, post-pregnancy pregnancy-compatible disease-modifying anti-rheumatic therapies (DMARDs) reduces maternal foetal mortality/ morbidity minimising complications. Objectives Service evaluation of the Combined Rheumatology Obstetrics held our hospital to evaluate patients rheumatoid against recommendations. Methods Retrospective was collected on interventions pregnancies 40 seen theCombined Rheumatology/ Obstetric Clinic from November 2020 February 2022 reviewing letters medical notes. Results 77% had arthritis including rheumatoid, psoriatic, juvenile idiopathic, axial spondylo-arthritis.15% connective tissue (CTD) lupus anti-phospholipid syndrome, Bechet’s undifferentiated CTD. 2%patients did not have formal diagnosis. 12.5% only pre-pregnancy counselling, 2.5% miscarriages. Of remaining 85% patients’ 10% obstetric care/ delivery other trusts. Treatment received by 12 months prior conception included DMARDs (37%), biologics (24%), combination (8%), steroids (10%). 25% were no treatment.During 29% oral DMARDs, 11% 16% needed steroids. 50% require any treatment. Interventions done medication review (100%), VTE assessments commenced prophylaxis (7.5%), change DMARD dosage (10%), additional added intramuscular/ intra-articular steroid injections (16%) physiotherapy referrals Maternal adverse include 5% preeclampsia, 0.07% gestational diabetes 0.02% pregnancy-induced hypertension growth restriction. 93.3% full-term deliveries. Only neonatal 6.6% preterm births. None congenital heart block. 3.3% malformations unrelated IRD/ therapy.Patient satisfaction surveys sent out 30% response rate. The questionnaire revealed all their consultation rheumatology consultants 100%satisfaction. 100% felt staff supportive they would recommend service friends family. 66.7% receiving clinic. 33.3% counselling 22.2% benefitted it. Conclusion Our rheumatology/ favourable mother baby high rates. offered appropriate timely improving outcomes. future aim is deliver this high-quality through robust referral pathways experience.
